HOW TO RETURN A LEASED MODEM
We do not accept the return of leased modems at our retail store. 

To return a leased modem, you will first have to cancel the CenturyLink service by calling 1-800-244-1111.

When you cancel your CenturyLink service, the customer service rep will mail you a "prepaid UPS return label." 

When you receive the return label in the mail, please stick in on a box and send the modem back to CenturyLink free of charge.

If you are moving, make sure you give them your new address. You may not receive a refund check on a deposit or your final bill may end up not getting paid.

Also make sure you save the tracking number on the return label so you have proof you sent it back.
